What is a chronic disease?

A ‘chronic disease’ is generally referred to as a condition that has been, or is likely to be present for at least six months, or is terminal. You may require treatment from a variety of health professionals on a regular basis therefore management of your chronic disease may require careful organisation.

There are a number of chronic diseases but they can include:

  • Arthritis
  • Cystic fibrosis
  • Asthma
  • Diabetes
  • Osteoporosis
  • Mental health conditions

We will work with you to provide for your individual needs through chronic disease management.

Upon diagnosis, you’ll be provided with both the emotional and physical support required and given advice on how to manage your chronic disease.

You’ll agree to a plan of action with your GP identifying exactly what your needs are, which services will be provided to accommodate for them, and what action needs to be taken in both the short and the long term.

It’s important to remember you may require the help of more than one medical professional.

Your GP will help to advise on the recommended professionals who can assist with your chronic disease management and help to ease any symptoms you may experience.
